In this role, the successful candidate will assist the RN in the care of patients and families by completing assigned patient care tasks, in accordance with the NYS LPN scope of practice and NYULH policy. In this role, the FGP LPN will provide competent, safe, and compassionate care with emphasis on an understanding of the challenges faced by health maintenance, illness, hospitalization, undergoing procedures, or diagnostic testing. The FGP LPN will check vital signs, recording a patient's medical history, and utilize specialized skills and safety when carrying out specific tasks of patient care as assigned, including phlebotomy, point of care testing via finger stick, with certification, and electrocardiograms.

Job Responsibilities

  • Participates in patient centered quality and performance improvement program consistent with service and departmental programs.
  • Acquires and maintains current LPN knowledge and competence to fulfill responsibilities.
  • Supports the missions, philosophy, standards, goals and objectives of NYULH.
  • Collects patient healthcare data systematically and, on an ongoing basis, using appropriate assessment techniques and instruments.
  • Enforces adherence of NYULH and FGP policies and procedures and standards including but not limited to HIPAA, Compliance, Human Resources, etc.
  • Participates in coordination of unit activities to provide patient care in a healing, humane and caring environment.
  • Engages in self-assessment and participates in activities to promote own professional growth and development.
  • Obtains patient vital signs, weights, EKGs as needed. Performs blood collection/phlebotomy as needed.
  • Administers medications, including injections in accordance with NYULMC clinical guidelines and NYS Scope of Practice.
  • Assists with maintaining patient flow to and from exam rooms and treatment areas. Assists in escorting patients. Ensure chart is available, with relevant lab work and test reports if indicated, for physician when the physician sees the patient.
  • Maintains an awareness of current patient centered evidence-based practices and uses knowledge of applicable research data in provision of patient care.
  • Collaborates with members of the interdisciplinary team and promotes collegial relationships.
  • Assists patients and communicates with licensed providers to obtain prescription renewals as needed.
  • Performs patient related functions as directed by the Licensed Provider and/or Registered Nurse (RN), and documents properly. These may include the following, as governed by LPN scope of practice: medication administration, vaccination, other injections, and patch placement, set up and/or assist for procedure.
  • Communicates directly with patients regarding their care or treatment using appropriate terminology and respectful manner. Interacts with medical staff efficiently. Interacts with support staff professionally.
  • Serves as a support and resource for nursing staff and providers.
  • Demonstrates knowledge of the NYUMC Service Standards and incorporates into the performance of responsibilities.
  • Functions as a respectful member of the health care team.
  • Ensures clinical compliance with Patient Safety initiatives and reporting.
  • Ensures clinical in-baskets are maintained and encounters are closed within a timely manner.
  • Considers needs and behaviors of specific patient age and cultural groups in all patient care activities.
  • Completes annual competencies such as Fire Safety, Hazardous Waste, Medsled, HIPPA compliance, etc.

  • Drives consistency in every patient and colleague encounter by embodying the core principles of our FGP Service Strategy CARES (Connect, Align, Respond, Ensure, and Sign-Off).
  • Greets patients warmly and professionally, stating name and role, and clearly communicates each step of the care/interaction as appropriate.
  • Works collaboratively with colleagues and site management to ensure a positive experience and timely resolution for all patient interactions and inquiries whether in person, by phone or via electronic messaging.
  • Proactively anticipates patient needs, and participates in service recovery by applying the LEARN model (Listen, Empathize, Apologize, Resolve, Notify), and escalates to leadership as appropriate.
  • Shares ideas or any observed areas of opportunity, to improve patient experience and patient access, with appropriate leadership. (i.e. ways to optimize provider schedules, how to minimize delays, increase employee engagement, etc.)
  • Partners with Patient Access Center and Central Billing Office team members to support collaboration and promote a positive patient experience.
  • Takes a proactive approach in ensuring that practice staff are fully versed in the Access Agreement gold standard principles.

Minimum QualificationsTo qualify you must have be a graduate of LPN program with current NYS OP LPN licensure and registration. Ability to complete multiple tasks efficiently and thrive in a team work environment which pursues a positive patient care experience. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed also required.
